I agree with your and the author's disentanglement of achievement and leadership, which I don't think is a particularly controversial point.

Here is my question, though: is achievement a necessary precursor to leadership? Or barring that, is it a very good predictor of leadership? To flip the question, can you have leadership without achievement, and is that common?

The human mind works by ignoring almost all of the information streaming into it. Focusing your attention is simply picking one thing to process and blocking everything else out. This is a roundabout way of saying that if there is any chance at doing something well in life, you have to accept the fact that you will have to fail in others. So, it all depends on what you define as 'it all'

Neil, I love this post. I believe the picture of hardship/failure followed by redemption is one of the most beautiful images in the world.

Personally, I believe persistence can only pay off if you have hope in the final goal. If one lacks direction, persistence may be in vain. This may help to answer your "how" - and you're correct. Living it right is hard.
